FIVE: Look for the ALL CAPITAL WORDS - AGE. GENDER. JOB/PATIENT. Fill them in, all capitalized with periods after. If you have trouble finding this, CTRL + F works wonders. Spell out the age, and for patients, it's the level your patient would be (high med low).
SIX: Scroll until you see the style tag for scrolling. After should be located the introduction of the actual template. Read through it and answering the following questions in the areas marked (FILL IN YOUR ANSWER). Simply erase that and put your answer there.
